To Our Patients:
To avoid any misunderstandings regarding fees and payments, we wish to present our practice policies concerning payment of treatment, and define the manner in which we address your dental insurance claims submission. We can no longer extend the courtesy of credit. Payment for services will be expected to be made at the time of treatment.
The following payment options will be offered:
Cash, Check or the following credit cards are accepted: VISA, MasterCard, American Express, and Discover.

- Prepayment allowance of 7% is offered for certain Periodontal Cases as presented in the treatment consultation.
- Payment plans for extensive Periodontal Treatment will be considered by request.
- Dental Implants Payment must be made at the surgical placement appointment with one of the following payment options:
- Payment in full by check, cash or credit card. (No 7% allowance)
- One half the fee paid at the placement appointment; balance paid in three consecutive monthly payments.

Claims for initial exams, x-rays, hygiene maintenance appointments and other non-surgical procedures will be sent electronically by us to insurance companies that require electronic submission. If your company cannot accept electronic claims, we will give you a paper claim for you to mail.
For surgical or non-surgical periodontal treatments which require record data such as x- rays, periodontal readings, we will send those claims by mail from our office. We also will require pre-estimation of periodontal treatments, in most cases, to avoid repeated duplication of office records and to inform you of what dental benefits your may expect.
We will accept assignment for dental insurance payments, only if a pre-estimation of benefits has been made prior to treatment, or if we are network providers in your dental insurance plan.
